From Egyptian Secrets of Albertus Magnus, Joseph H. Peterson edition:
For Toothache.
When a person is troubled with toothache, the teeth are decayed and hollow, take cornelian cherries and wax, make little cakes thereof, and put them upon a heated iron, cover them over with a pot, which has a hole in the bottom, let the vapors rise through a funnel into the mouth, and the worms will fall out of the teeth.
The grimoire Egyptian Secrets of Albertus Magnus lists this spell.
